Concerta and Priapism
Concerta contains methylphenidate, the same active ingredient in Ritalin and many other ADHD drugs. From 1997 through 2012, this class of drugs has been associated with 15 cases of priapism — a painful, long-lasting, and unwanted erection that is not due to sexual arousal, and can cause permanent erectile dysfunction. Of the 15 cases, 12 occurred in children (median age 12.5 years).
The FDA published a Drug Safety Communication and recommended that parents and caregivers of male children on Concerta should talk about the symptoms of priapism and what to do if they occur.
Children who do not recognize symptoms or are too embarrassed to tell an adult can potentially develop severe tissue damage in the penis, which may need to be treated with hospitalization and/or surgery. Without treatment, priapism cause lifelong erectile dysfunction (impotence).

What is the problem?
